The guideline for resolution is somewhat subjective. Anything with length or width 600 or longer is generally considered to be too large (though there may be exceptions if it's, say, 25x600). Anything 400x400 or smaller is considered to be sufficiently small. Anything in between is somewhat subjective. The summary you provided looks very good; it has the name of the game and the copyright holder, which are the two required things. Providing a link like you did is also helpful, and you may want to specify that you scanned the image (though that's by no means required).
The only thing missing is a fair use rationale. Essentially, it's just an explanation of why the image meets the fair use criteria for being used in one or more articles. Here's an example of an image I uploaded today that has a fair use rationale. Fair use rationales are technically required for use in articles, but it's usually overlooked when you have, say, a game cover being used in an article about the game like in this situation. However, if you were going to use the image in another article, say, about the company that produced the game, it's more likely that someone will ask for a fair use rationale. I'll go ahead and add a fair use rationale for Image:Bangai-ON64Box.jpg; if you upload any more game covers, you can more or less copy and paste it.
