Alright, let's review this road!

"Between Boonville and Lowville, it follows the Black River Valley and between Watertown and Morristown, it follows the St. Lawrence River valley." - I suggest splitting this sentence, it's kinda long
The lead is four paragraphs of very different lengths. Could you make it two or three more evenly spaced paragraphs?
"NY 12 overlaps NY 28" - should you link concurrency here?

"NY 12 also connects with Interstate 81 (I-81) near both its beginning and end points in the same manner. " - the last part seems redundant
Much of "Route description" is unsourced.

" NY 12 enters the city of Norwich, the largest settlement on the route in the county, and becomes South Broad Street" - this seems like it would need an independent source from much of the route description

"Broome and Chenango counties" - I feel like the first paragraph is a bit wordy, like it could be broken up into four or five sentences.
"A major project in Utica currently exists for the repair of the Arterial entrance. This project is expected to be completed in 2015. " - it's 2020
"as it heads north through the narrow southeastern portion of the county (with a width of only 10 miles or 16 kilometres). " - is the county or the route 10 miles?
"This configuration remains through the villages of Barneveld (where NY 28 joins NY 12 south of the village and NY 12 connects to NY 365 via an interchange) and Remsen to the vicinity of Alder Creek in the town of Boonville, where NY 28 leaves NY 12 via a partial trumpet interchange and the road narrows to two lanes shortly afterward. " - this should be split
"3 miles (5 km) outside of Lowville at West Lowville, NY 12 leaves its due west alignment and curves to the northwest, with the westerly alignment continuing onward as NY 177." - WP:MOS says you shouldn't start a sentence with a number. That can be confusing for non-English natives. Any way you could reword this?
"10 miles (16 km) north of NY 180, NY 12 reaches the village of Clayton on the St. Lawrence River." - ditto above
Is it worth mentioning how close to the Canadian border the route ends? Not sure if it's trivial, but I was curious
I notice you don't mention NY 12F's intersection with NY 12 in the route description.

@Hurricanehink: It doesnt say it exactly, but it shows it as obviously the biggest by looking at the map of the county. I myself didnt add this sentence to the article.
