Thanks for uploading File:Charles denham.jpg. I noticed that the file's description page currently doesn't specify who created the content, so the copyright status is unclear. If you did not create this file yourself, you will need to specify the owner of the copyright. If you obtained it from a website, then a link to the website from which it was taken, together with a restatement of that website's terms of use of its content, is usually sufficient information. However, if the copyright holder is different from the website's publisher, their copyright should also be acknowledged.

As well as adding the source, we also need to know the terms of the license that the copyright holder has published the file under, usually done by adding a licensing tag. If you created/took the picture, audio, or video then the {{GFDL-self}} tag can be used to release it under the GFDL. If you believe the media meets the criteria at Wikipedia:Non-free content, use a tag such as {{non-free fair use in|article name}} or one of the other tags listed at Wikipedia:Image copyright tags#Fair use. See Wikipedia:Image copyright tags for the full list of copyright tags that you can use.

Image problems[edit]

Please stop uploading images for which you fail to add the appropriate needed details, or whose details you don't have. We take copyright status and sourcing very seriously and the images you uploaded don't have this information, so have been nominated for deletion. If you did not take the photos yourself and they are copyright to someone else, or you found them on a website, please don't upload them unless you have permission of the copyright holder. We also need to know the source website or publication so we can check any copyright tag you add to an image. You may find it useful to read my image copyright information page. Thanks for uploading File:Charles R. Denham, MD.jpg. I noticed that while you provided a valid copyright licensing tag, there is no proof that the creator of the file agreed to license it under the given license.

If you created this media entirely yourself but have previously published it elsewhere (especially online), please either

make a note permitting reuse under the CC-BY-SA or another acceptable free license (see this list) at the site of the original publication; or
Send an email from an address associated with the original publication to permissions-en@wikimedia.org, stating your ownership of the material and your intention to publish it under a free license. You can find a sample permission letter here. If you take this step, add {{OTRS pending}} to the file description page to prevent premature deletion.

Please note that Wikipedia takes copyright and copyright infringement very seriously. Images and other media may only be uploaded and included if they meet the conditions stated in our image use policy, and if their provenance is clearly documented. Dr. Denham is Editor-in-chief of a global journal and very careful about conflict of interest. His mission is patient safety and he is not promoting sales of a product. Can you give an explanation as to why we were blocked?

Decline reason:

From your own words below "I...was told, along with a colleague who is no longer here, by Dr. Denham to create a page for him and the company". Therein lies the problem. Wikipedia's has a policy regarding neutrality; editing or creating articles regarding a subject with which you are so closely associated is strongly discouraged as it is nearly impossible to write with the dispassionate prose required by policy. If your sole purpose in creating an account is to add content regarding your supervisor/company, that is a pretty clear sign that you will ultimately run afoul of WP:NPOV. Wikipedia article subjects are chosen by volunteer editors based on notability; if TMIT/HCC and Dr. Denham meet the criteria for inclusion, it is likely an uninvolved neutral editor will ultimately create an article. Sorry for not understanding what I'm supposed to do. I am a researcher/editor at TMIT/HCC [Dr. Denham's company] and was told, along with a colleague who is no longer here, by Dr. Denham to create a page for him and the company.
