中文(臺灣):民國80(1991)年11月15日下午4點,南下經過苗栗縣造橋鄉的一輛莒光號列車於山線134號誌站轉換軌道進入副線時,一輛1006次EMU100型自強號正北上行駛,兩輛火車本該分道。但在莒光號的前5節車廂轉向後,剩餘的3節車廂被自強號以時速85公里撞上。莒光號、自強號車廂翻覆,造成30死112人輕重傷的嚴重事故,故列為苗栗大事件之一。 當時火車對撞的車禍現場,自強號車頭撞毀,鋼板整片被削掉。原本在自強號睡覺的乘客,醒來在莒光號裡面。許多罹難者飛出車外,血肉模糊,難以辨識。自強號駕駛員蘇金焜雙腿折斷。鐵路局認為,事故原因是司機員沒有減速、不遵守號誌規則的人為疏失。事實上,蘇金焜曾在發車前反映「列車自動停車裝置」(ATS/ATW)故障,台鐵依舊堅持發車,且134號誌站被通報故障數週台鐵仍未維修,這兩者台鐵都沒有列入後續報告裡。檢方調查發現,ATS/ATW曾有故障紀錄,根據鐵路法規定,台鐵應該進行檢修,有採取防止措施的義務卻無作為,進而引發重大車禍。時任鐵路局局長陳世芳、機務處處長蕭輝煌被認為是刑法「不作為犯」,被檢方提起公訴。陳世芳和蘇金焜被以業務過失致死罪起訴。蘇金焜認為台鐵事故報告有失公平,但認為自己有道義上責任,所以被判刑4年有期徒刑時無異議。陳世芳則被判無罪。
