The ongoing worldwide pandemic of coronavirus disease 2019 (COVID-19), a new infectious disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), was confirmed to have reached Sweden on 31 January 2020, when a woman returning from Wuhan tested positive. Starting from 26 February, following outbreaks in Italy and in Iran, multiple travel-related clusters appeared in Sweden. Community transmission was confirmed on 9 March in the Stockholm region. Since then, individuals in every län (county) have tested positive for COVID-19. The first death was reported on 11 March in Stockholm, a case of community transmission.
Sweden has, unlike many other countries, not applied any lockdown, with most measures being voluntary. The Swedish constitution mandates that the relevant expert agency, the Public Health Agency, rather than politicians must initiate any actions to prevent the virus. On agency recommendation: the government has limited freedom of assembly, temporarily banning gatherings of over 50 individuals and physically closed secondary schools and universities. Primary schools have remained open, in part to avoid healthcare workers from needing to stay home with their children.
The Public Health Agency and government issued recommendations to work from home, if possible; for people above 70 to stay at home; and issued general recommendations to avoid unnecessary travel within the country and to keep a social distance between people. Those with even minimal symptoms which could be COVID-19 are recommended to stay home. The 'karensdag' or initial day without paid sick-leave has been removed by the government and the length of time one can stay home without a doctor's note has been raised from 7 to 21 days.
As of 2 April 2020[update], there were 5466 confirmed cases, of which 429 received intensive care, and 282 deaths related to the newly found virus in Sweden, with Stockholm County being the most affected.[1]
History
The COVID-19 outbreak, which the World Health Organization (WHO) declared a pandemic on 11 March,[2] has had a disparate impact on different areas of Sweden. The first case was introduced directly from Wuhan to the country on 24 January, with a woman confirmed positive for the virus on 31 January in Jönköping. The case was fully isolated and there are no reports of further spread.[3][4]
The second case was introduced from northern Italy on 26 February.[5][3] With five additional cases confirmed on the 12th, the Swedish Public Health Agency ('Folkhälsomyndigheten') put out a statement that these cases were all related to travel to high-risk zones and that there was no evidence of community transmission.[6] Disease control measures, including extensive contact tracing turned up over 200 travel-related cases in the following weeks, all with connection to confirmed cases or travel to high risk regions.[7]
On 9 March the first cases of community transmission was confirmed among two patients who had sought care at S:t Göran Hospital on 6 March.[12] They were assumed to have been infected through community transmission approximately 7 days before seeking care.[citation needed]
The Public Health Agency of Sweden responded on the 10th by raising risk assessment of community spread from moderate to very high, which is the highest level.[13][13]
The first death was reported on 11 March, when a person in their 70s from the Stockholm region died in the intensive care unit of Karolinska University Hospital. The person was reported to have acquired the virus through community transmission, believed to have occurred about one week before death. The person also belonged to a risk group.[14]
Freedom of assembly has been limited to gatherings of less than 50 people, through the passage of and application of new law on 11 March. The initial limit was 500, lowered on 27 March.[15][16][17]

Policy
The Swedish government has tried to focus efforts on encouraging the right behaviour and creating social norms rather than mandatory restrictions. Government officials including Swedish prime minister Stefan Löfven, as well as the Public Health Agency of Sweden have encouraged each individual to take responsibility for their own health and the health of others.[19] Löfven has stated that stricter measures will be taken as appropriate, and as guided by the expert agencies.[20]
The government has issued progressively stricter advisories against travel. Unnecessary travel to certain parts of China was recommended against the 17th February,[21] Iran the 2nd March,[22] Italy the 10th of March[23] and all international travel was recommended against the 14th of March.[24] Travel from non-EU/EES member states was stopped the 17th of March[25] and unnecessary travel within Sweden was advised against the 19th of March.[26]
Rules and legislation
On 11 March, the same day as the first Swedish death to COVID-19, the Swedish government passed a new law, temporarily banning all gatherings larger than 500 people, with threat of fine and prison.[15] The ban will apply until further notice.[16] On 27 March the government announced that gatherings of more than 50 people were banned.[17]
On 11 March 2020, the karensdag, the unpaid first day of sick leave, was temporarily discontinued in an effort to encourage people to stay home if they were experiencing symptoms that are consistent with COVID-19.[27]
On 24 March 2020, the government introduced new restrictions to bars and restaurants requiring all service to be table service only. Venues that do not adhere to the new restrictions could be shut down.[28]
Economics
The normal costs of employer contributions have also been temporarily discontinued for small business owners. This will save small businesses approximately 5000 SEK per employee each month but will result in a loss of tax revenue of 33 billion SEK.[29]
Local governments, such as the municipal government in Gävle, have applied measures to businesses delaying the payment of invoices until September 1, 2020 at the earliest and deferring rent payments.[30] On 2 April, the Financial Supervisory Authority ('Finansinspektionen') decided that Swedish banks temporarily can allow exemptions for housing mortgage lenders regarding amortizing of loans.[31]

The increasing number of cases in large areas such as Stockholm and Uppsala has resulted in the cancellation or postponement of up to 90% of planned surgeries, including cancer related surgeries.[32]
Staffing
On 25 March 2020, Björn Eriksson, the Director of Healthcare in Stockholm, appealed to anyone in the Stockholm region who had experience in healthcare to volunteer. As of the 26 March 2020, 5100 people with healthcare experience had registered as volunteers.[33]
Beds
The Stockholm International Fairs, Stockholmsmässan, are being converted into a field hospital with the help of the Swedish Defence Forces. The field hospital will be able to house 600 seriously and critically sick patients.[34] The Swedish Defence Forces will provide equipment for 30 of the 600 beds and the Stockholm Regional Council will provide the remaining 570.[34]

On March 13, 2020 the April Swedish Scholastic Aptitude Test ('Högskoleprovet'), was cancelled affecting approximately 70 000 prospective students who had registered themselves. This was the first time the Swedish Scholastic Aptitude Test has been cancelled since it was established in 1977.[36]

The Almedalen Week, considered to be the biggest and most important forum in Sweden for seminars, debates and political speeches on current social issues, held in Visby every summer,[41] was cancelled as a result of the ban on large gatherings. The decision was taken on 1 April by the organiser after consultation with the major political parties.[42] Prime Minister Stefan Löfvén had already announced that he had cancelled his planned participation in the upcoming event.[43]
Culture
Starting 30 March 2020 the public library in Gävle will start with a book delivery service for people aged 70 or older. The library will also start a take-away service where you can pre-loan books and pick them in a take-away bag.[44]
Sports
The world's largest half marathon in Gothenburg, Göteborgsvarvet, was postponed until later in 2020 and then cancelled completely on 27 March.[45]
On 19 March, the governing body for association football in Sweden formally announced that the premiere of the 2020 season for the first and second division leagues, men's Allsvenskan and Superettan as well as women's Damallsvenskan and Elitettan, will be postponed to late May or early June. The decision will not affect the leagues below the second level.[46] Two days later it was announced that the 2020 edition of the association football award ceremony Fotbollsgalan was cancelled.[47]
The annual recreational bicycle race Vätternrundan, scheduled to be held in June, was also cancelled as a result of the pandemic. The organisers made the decision public on 2 April.[48][49]
International response
On 8 March Vietnam suspended its visa waiver program to Sweden and seven other European countries in response to the number of cases.[50]

With Sweden having taken some of the least strict measures of any European country or of countries with a similar number of cases, the response has been both criticized and praised by international media. Additional data
As of 27 March nearly 3 out of 4 patients in intensive care in Sweden are men. The majority of them have underlying symptoms such as high blood pressure. The number of deaths in Sweden is also disproportionately men.[59]
It has also been reported that a disproportionate number of those that have passed away have been Swedish Somali, with 6[60] out of 89[61] deaths being members of the Swedish Somali community in the Stockholm region.[60]

A woman in her 20s, returning from Wuhan on 24 January, tested positive for the SARS-CoV-2 virus and was admitted at Ryhov Region Hospital in Jönköping.[197][3] Having isolated herself voluntarily at home upon arriving in Sweden she was deemed not to have posed any transmission risk during travel or during her initial days in Sweden.[3] The local university college of Jönköping, stated at the time that it had around 200 Chinese exchange students, responding that the woman may have been a student.[198][199] The woman was declared asymptomatic, but not fully discharged from healthcare services after over one month of care on 3 March.[4] However, viral fragments continued to be detectable with transmission of live viral particles being deemed unlikely.[4] |

The Swedish Ministry of Foreign Affairs advised against all non-necessary trips to northern Italy, specifically the regions of Piemonte, Liguria, Lombardia, Emilia-Romagna, Trentino-Alto Adige, Valle d'Aosta, Veneto, Friuli Venezia Giulia, Marche and Toscana.[226] Turin, Milan, Venice, Verona, Trieste and Florence are large cities in these regions. The Public Health Agency of Sweden, who initiated the recommendation for the Swedish Ministry of Foreign Affairs, states that the decision is based solely on the strain of the Italian health care system.[227] The same day, Swedish National Broadcaster SVT held a crisis meeting to consider broadcasting the live finals of Melodifestivalen 2020 on 7 March without an audience, as a response to the growing outbreak. The Danish equivalent had recently decided to broadcast Their version of the finals without an audience. Ultimately, SVT decided to allow the audience to enter the arena, although they advised people who felt sick to stay at home.[228] | ||

To date, Stockholm region states that of the 115 confirmed cases in the region, 101 people have been infected in Italy or through contact with a person who has visited Italy, 8 cases have been infected in Iran or through contact with a person who has visited Iran, one person has been infected in another European country, and five cases are still under investigation.[120] One of the Örebro cases confirmed this day is believed to have caught the virus on a trip back from the US, which he/ she arrived back from approximately a week ago. The other two persons are related to this person. Contact tracing has begun around this case,[128] which is the third case of transmission to Sweden from a non-risk area. The Uppsala case is a senior high school student at Celsiusskolan in Uppsala.[122] The Stockholm region press office did not report cases detected on the afternoon of the 8th, instead reporting an additional 32 cases on the 9th.[233] One of the Örebro cases is reported to have visited an open house at the Örebro University on 4 March. Örebro region advises any attendees to be attentive to if symptoms arise.[234] |
A preschool in Hisings Backa, Gothenburg, decided to close for two weeks due to an infected person having been on the premises. The preschool has not said whether the infected person is a child or one of the staff. Children and staff have all been ordered to quarantine themselves at home by Smittskydd Västra Götaland, and the preschool is undergoing a deep clean. The preschool has five staff.[235]
On the same date, approx. 15 health care staff at Danderyd Hospital were quarantined after having been in contact with a patient on 5 March who arrived by ambulance to the emergency unit. The patient had not been to a high-risk area and was not initially suspected to have COVID-19, but when the patients health deteriorated hospital staff decided to test the patient for the virus, which showed the person was infected.[236] Vietnam suspends its visa waiver program to Sweden and seven other European countries in response to the number of cases.[50] | ||
9 | Five cases were confirmed in Halland (the first in the region),[129] one in Värmland,[127] seven in Västra Götaland,[237] two in Jönköping,[134] two in Uppsala,[135] and 32 in Stockholm (all of which had been confirmed the previous day).[233][233][133] A first case of Covid-19 was also reported in Sörmland,[238] a first case in Norrbotten,[142] five first cases in Västerbotten[144] and one first case in Östergötaland.[146]
The first Halland case is an elementary school student at Särö school in Kungsbacka who had travelled to northern Italy.[239] On the same date, Värmland had a total of 21 cases, of which 20 had visited northern Italy, and 19 of these were connected to the same bus tour. One case has been infected in the Netherlands. One of the Värmland cases is a teacher at Norrstrandsskolans junior high school.[111] One of the 21 cases is admitted to Karlstad hospital. The majority of the cases are from the Karlstad area.[127] Further, six new cases were confirmed in Västra Götaland, of which two are in the Skaraborg area.[132] On the same date, the first community transmissions were detected in Sweden. Two persons in the Stockholm area who had no known connection to any of the risk areas or an infected person, were found to be infected, both patients at St. Göran Hospital since Friday 6 March in two different wards.[240] Both cases have been transferred to Huddinge Hospital for further investigation.[241] One of the cases in Västra Götaland was a patient at Kungälv Hospital with signs of respiratory symptoms since approx. a week who was tested for Covid-19 after the decision of the Public Health Agency of Sweden on 4 March. This patient was tested on Sunday 8 March and confirmed positive a day later. The patient was immediately transferred to Sahlgrenska University Hospital and the affected ward at the Kungälv Hospital has been closed for patient admission. Highly exposed staff at Kungälv Hospital have been told to stay home for a while, and contact tracing has begun.[143] |
The Health Care Management ('Hälso- och sjukvårdsledningen') in Värmland decided that all health care staff that have visited northern Italy or other high-risk areas (current and future as announced by the Public Health Agency of Sweden) are to self-quarantine for 14 days due to vulnerable patients they may meet in their line of work. The decision is also based on incidents of transmission from health care staff to patients in China, Norway and the United States. An estimate of the number of affected health care staff was not publicly available by this date. Where possible, affected staff are encouraged to work from home and in cases where this is not possible the staff are suspended from work on medical grounds but will maintain salary benefits.[242]
On the same date, approx. 30-40 health care staff at St. Göran Hospital in Stockholm were quarantined after having cared for a Covid-19 infected patient. Another patient at the same hospital has been tested and is awaiting results. The hospital immediately went into a state of readiness to handle the situation.[241] | ||
10 | Three cases were reported in Scania,[243] 60 in the Stockholm region,[244] 15 in Västra Götaland,[152] two first cases in Sörmland region,[154] one first case in Jämtland[245] and first three cases in Västernorrland region.[246] Stockholm region announced this day that two of the 207 infected cases in the region were in intensive care.[244] One of these patients is a patient from the intensive care unit at Södersjukhuset Hospital whom was confirmed infected on Monday 9 March, and immediately moved to Karolinska University Hospital for further isolated intensive care. The patient has relatives whom have recently travelled in Europe, but the hospital announces it is too early to determine whether the patient has been infected by this person. Contact tracing has begun and Södersjukhuset Hospital has gone into a state of readiness to handle the situation and are trying to identify people who may have been exposed to the virus.[247] In the Stockholm region, a total of ten patients have been hospitalised.[248] One of the Stockholm cases is a judge at the Svea Court of Appeal who was infected on a trip abroad, although not a risk area. Due to this, the court has told 25 staff to self-quarantine and some hearings have been postponed.[249]

The Södersjukhuset Hospital in Stockholm went into a state of readiness after an ICU patient was confirmed to have Covid-19. The patient was transferred to Karolinska University Hospital for further isolated intensive care. It is not known whether the patient got the virus from a relative traveling in Europe and whether the patient has infected staff or patients. Approx. 70 staff have been in contact with the patient but the hospital administration has decided to allow them to continue working until they show symptoms of infection.[250] This decision has received internal criticism from staff of not being forceful enough.[251]
On the same date, the Public Health Agency of Sweden raised the level of risk of the virus spreading from "high" to "very high", which is the highest level. This comes after news that there are indications of ongoing local transmission in the Stockholm and Västra Götaland area.[252][253] At the same time, the agency advised everyone with respiratory infection, even mild ones, to refrain from social contacts in private and working life- where there is a risk of spreading the virus. They also ask health care staff in nursing homes and caring of elderly to not work if they have any symptoms of respiratory infection. Relatives of elderly should also avoid unnecessary visits at hospitals and in facilities for elderly and never visit if there are any respiratory symptoms.[254] After this announcement, Blekinge Health Care Region enforced a total ban on visits by family members and friends at health care and elderly establishments and hospitals in the region.[255] Region Värmland enforced a similar ban on all visits to hospitals and health care establishments in the region.[256] After the announcement of the Public Health Agency of Sweden to protect the elderly and the frail, Stockholm Region decided to change their strategy for testing for COVID-19 to mainly focus on the very ill and not just people who had visited risk areas.[257] Danderyd Hospital also went into a state of readiness due to the situation of the Covid-19 spread in Stockholm.[258] | ||

The same date, Södertälje Hospital went into a state of readiness due to the situation in Stockholm region.[257] The government has banned all gatherings larger than 500 people, under threat of fines and a six-month prison sentence.[15] The same date, the government announced that the qualifying day of sickness ('karensdag') will be temporarily abolished in order to ensure that people feeling slightly ill will stay at home from work. This means that the state will pay sick pay allowance from the first day the employee is absent from work. The decision is effective as of 11 March 2020 and is believed to reduce the virus spreading as well as the burden on the health care system.[260] | ||
